Return 400 for milestone titles and notes that are too long

POST /projects/{projectId}/milestones and PATCH /projects/{projectId}/milestones/{milestoneId} now reject a title or note longer than the underlying database column with a 400 Bad Request and a message naming the maximum length, instead of failing with a server error. The maximum lengths are also documented in the OpenAPI schema for both fields.
